Identificación de micobacterias ambientales, aisladas de distintas fuentes de agua, mediante análisis de patrones de restricción y su comparación con métodos fenotípicos

Gran parte de las micobacterias ambientales (MA) son consideradas patógenas oportunistas responsables de micobacteriosis tanto en huéspedes inmunocomprometidos como inmunocompetentes. No existe evidencia de transmisión directa entre individuos siendo el medio ambiente la fuente de infección. En muchos laboratorios se ha reemplazado la identificación bioquímica de las MA por nuevas técnicas moleculares rápidas y menos complejas que la tipificación clásica. Entre los métodos moleculares el PRA (PCR-Restriction Enzime Analysis) se ha descrito como una técnica de rutina capaz de ser implementada en los laboratorios de baja complejidad. El objetivo principal de este trabajo fue valorar la utilidad de la técnica del PRA en la tipificación de MA recuperadas de muestras de agua. A los 56 aislamientos obtenidos, provenientes de humedales, agua de red y de fuentes arquitectónicas públicas, inicialmente se los identificó mediante pruebas bioquímicas y el análisis de los patrones de restricción, obteniéndose una concordancia del 48,21 por ciento. Posteriormente se les realizó secuenciación de un fragmento de los genes 16S rARN y hsp65. El PRA resultó ser más preciso cuando se lo comparó con los resultados obtenidos por secuenciación del gen hsp65 que cuando se los contrastó con la secuenciación del fragmento 16S rARN, obteniéndose una concordancia del 50 por ciento y 35,71 por ciento, respectivamente. El PRA es una técnica rápida y fácil de implementar a diferencia de las pruebas bioquímicas, siendo necesario recurrir a técnicas moleculares complementarias, específicamente cuando se desea identificar a aquellos aislamientos de origen ambiental que presentan colonias pigmentadas y de rápido crecimiento, así como también cuando se evidencian posibles nuevos patrones de restricción. Las pruebas bioquímicas son de gran utilidad para respaldar los datos obtenidos por técnicas moleculares, principalmente en aquellos casos en los cuales por sí solos, tanto uno u otro método, no pueden definir la especie de Mycobacterium aislada. Las distintas fuentes de agua estudiadas constituyen un importante reservorio de MA, posibles patógenas oportunistas, de importancia para la salud pública.
