Presentación del síndrome de malformación occipital caudal en un canino.
Resumen
Se presentóa la consulta un canino derazachihuahueñode 3 meses de edad con Ataxiahiperestesia, alodinia yrascado fantasma (tendencia a rascarse en la región cervical, hombro o cara sin que llegue a haber contacto). Se realizó hemograma, urianálisis y serología los cuales arrojaron resultados normalesy fueronnegativo aToxoplasmosisyNeosporosis. Se le indicó una radiografía de la región craneal, bajo sedación, con incidencias radiográficas oblicuas rostrodorsal y caudo-ventral del cráneo,observándose una malformación sobre el hueso occipital. El síndrome de malformación occipital caudal es una entidad clínica derivada de una malformación de la fosa craneal caudal que puede ocasionar protrusión cerebelar. Cuando el cerebelo, órgano encargado de controlar el equilibrio y la coordinación neuromuscular, desciende a través del foramen magnumy comprime el tronco encefálico provocando una herniación cerebral de la parte más caudal de este a través del foramen magno.La salida del cerebelo, y a veces incluso del tronco del encéfalo, pueden producir alteraciones en la dinámica de flujo normal del líquido cefalorraquídeo, llegando a originar cavidades denominadas siringomegalia. Esta patología es depronósticoreservado ya que depende de laevolución de lalesión medular progresiva. Se instauróel tratamientomédicocon prednisolona 10 mg/Kg mejorando inicialmente la sintomatología se indicó realizarel tratamiento quirúrgico sugiriendo la técnicade craniectomía subtotal occipital con durotomía A 3-month-old chihuahuadog presented to the consultation with ataxia hyperesthesia, allodynia and phantom scratching (tendency to scratch the cervical region, shoulder or face without contact). Hemogram, urinalysis and serology were performed, which yielded normal results and were negative for Toxoplasmosis and Neosporosis. An X-ray of the cranial region was indicated, under sedation, with oblique radiographic incidences of the face-dorsal and caudo-ventral part of the skull, showing a malformation on the occipital bone. Caudal occipital malformation syndrome is a clinical entity derived from a malformation of the caudal cranial fossa that can cause cerebellar protrusion. When the cerebellum, the organ responsible for controlling balance and neuromuscular coordination, descends through the foramen magnum and compresses the brain stem, causes brain herniation of the most caudal part of the brain through the foramen magnum. The output of the cerebellum, and sometimes even of the brainstem, can produce alterations in the normal flow dynamics of the cerebrospinal fluid, leading to the origin of cavities called syringomegaly. This pathology has a reserved prognosis since it depends on the evolution of the progressive spinal cord injury. Medical treatment with prednisolone 10 mg/Kg was established, initially improving the symptoms. Surgical treatment was indicated, suggesting the technique of subtotal occipital craniectomy with durotomy
