Reparación de la hernia diafragmática experimental en el perro mediante el empleo de un flap muscular obtenido del músculo recto abdominal

Se describen la técnica y los resultados obtenidos de la corrección experimental de la hernia diafragmática mediante la utilización de un flap muscular obtenido del tercio medio craneal del músculo recto abdominal. Se emplearon 15 perros (n=15) divididos en tres grupos tratamiento integrados por 5 animales cada uno (n=5), los grupos tratamientos difirieron entre sí por el tiempo de duración de los post-operatorios (30, 60 Y 90 días) al cabo de los cuales fueron sacrificados para realizar estudios macro y microscópico de los resultados quirúrgicos. A los animales de los tres grupos se les practicaron defectos rectangulares de 4 x 5 cm. en el diafragma en tanto el flap alcanzó al diafragma por la cara torácica haciéndolo penetrar la cavidad torácica a través del 8° espacio intercostal. Los bordes del defecto diafragmático y del flap fueron suturados con puntos simples utilizando material no absorvible. En las necropsia se observaron los aspectos macro y microscópicos del flap, características de la unión del flap con el diafragma como así también las características de las superficies de contacto con las serosas. Clínica y radilógicamente los animales no mostraron alteración alguna observándose en la necropsia la reparación del defecto que presentaba ligeras adherencias de vísceras abdominales (hígado), y ninguna en la cara torácica. Macroscópicamente los flaps presentaban la superficie de color blanco y al corte se obervó el aspecto muscular. Microscópicamente consistió en tejido muscular conteniendo tejido de granulación. Podemos concluir que la técnica es factible de ser utilizada con éxito en defectos diafragmáticos ventrales o colaborar con otras técnicas de reparación diafragmática. The researches describe the technique used and the results achieved in the experimental correction of a diaphragmatic hernia using a muscular flal obtained from the third cranial medium ofthe m. rectus abdominis. 15 dogs (n=15) were used divided into 3 groups oftreatment, each group with animals (n=5). The difference between the treatrnent groups was post-operative period (30, 60, 90 days) after wich the animal's were killed to make macroscopic and microscopic studies of surgical results. Rectangular defects of 4x5 cm were made in the diaphragm of all animals. Tha flap reached the diaphragm through the thoracic side surface entering to thoracic cavity between the 8 thand 9 th ribs. The edges of diaphragmatic defect and the flap were sutured with simple stltirres using nonabsorvible material. During the necropsy the researches verified the macroscopic and microscopic aspects of the flap, the characteristics of the attachment of the flap and the diaphragm, and the characterists the contact surfaces with the serous surfaces. The animals showed no alterations clinically or through X-rays. The repair of the defect was evident in the necropsy, showing slight adhesions of abdominal viscera (liver) but no adhesions on the thoracic surface. Macroscopically, the flaps surface was sligtly white and the muscular aspect was observed when the flap was cut. Microscopically it consisted of muscular tissue wich contained granulation tissue. The researches concluded that this is a feasible technique wich can be used in the correction of ventral diaphragmatic defects or wich can contribute to other techniques.
