Metabolismo lipidico postprandial y riesgo cardiovascular en pacientes con hipotiroidismo

En los últimos años nos hemos ocupado de estudiar pacientes con trastornos del metabolismo lipídico. Se determinó que algunos de los motivos son la dieta predominante con ácidos grasos saturados y la falta de ejercicio físico, acompañado por hipertensión arterial, sobrepeso u obesidad. Otro factor importante es el hipotiroidismo, el cual es el motivo del presente estudio. Se plantea determinar la relación entre la lipemia postprandial en el hipotiroidismo en diferentes estados de la disfunción tiroidea según valor de TSH. Caracterizar y analizar factores ambientales como sedentarismo y tabaquismo. Informar a la población de la importancia de éste estudio. Se midió lípidos basales y postprandiales en mujeres con hipotiroidismo (n=31) vs control (n=24). Se separó según valor de TSH (A1.1: TSH < 2 µUI/ml, A1.2: TSH 2 - 4.2 µUI/ml, A2: TSH > 4 µUI/ml y B: control), luego de una sobrecarga alimentaria se determinó el aclaramiento a las 2 y 4 hs. Se observó diferencia significativa (DS) en el estado basal de LDL-C y TG entre los grupos estudiados. En el estado postprandial se encontró DS en LDL-C a tiempo cero y el TG a tiempo 122 y 240 min. Una vez separados en grupos según valor de TSH, en el estado basal, se encontró DS en CT, LDL-C y TG entre el grupo A1.1 vs control. Los pacientes hipotiroideos tienen mayor prevalencia de poseer valores de lípidos mayores principalmente en LDL-C y TG, implicando el riesgo de producirse lesiones e inflamación en paredes arteriales, dando lugar a la aterosclerosis.
