Capacitación del personal de los servicios de atención de la madre y el niño y el trabajo en conjunto para una lactancia eficaz

Una lactancia exclusiva eficaz disminuye ingresos por alimentación deficiente incrementando su prevalencia hasta el sexto mes. Objetivos: Comparar resultados obtenidos antes y después de la implementación de la Estrategia Hospital Amigo y analizarla en el Hospital Dr. Lucio Molas. Metodología: 1) trabajo analítico comparativo, retrospectivo de neonatos internados en Neonatología (años 2006 y 2008) por alimentación deficiente. 2) Encuestas al equipo de salud, puérperas y madres de niños hasta un año. Resultados: En 2008 ingresaron 128 neonatos por alimentación deficiente, 8.6% (11) por ictericia, 10 menos que en 2006. De 50 encuestas al personal de salud, 90% conocía la Estrategia, 72 % realizó curso de Lactancia y 40% manifestó que hay barreras. De 49 puérperas, 57% recibió información sobre lactancia, 77 % saben cuidar los pezones, pero no la frecuencia de mamadas. La lactancia materna exclusiva en menores de 6 meses fue 11.11%. Conclusiones: Desde que se implementó la Estrategia en el Hospital Dr. Lucio Molas se redujeron los ingresos neonatales por ictericia por leche humana. El 72% del recurso humano está capacitado para lograr una lactancia eficaz. La discontinuidad de la capacitación en 2009 se nota en existencia de agentes no capacitados. Esto contribuye a mantener la resistencia del personal a la implementación de la Estrategia y se ve reflejada en incumplimiento de las metas de lactancia materna exclusiva al sexto mes. Exclusive and effective breasfeeding reduces admission due to poor feeding increasing its prevalence up to the sixth month. Objectives: Comparing the results obtained before and after the implementation of the Baby Friendly Hospital Initiative and analizing it at Dr. Lucio Molas Hospital. Methods: 1) An analytic, comparative and retrospective study of neonates hospitalized in Neonatology in 2006 and 2008 due to poor feeding. 2) Enquiries to the health personnel, postpartum women and mothers of infants less than one year of age. Results: In 2008, 128 neonates were admitted due to poor feeding -8.6% (11) due to jaundice- 10 less than in 2006. Out of 50 enquiries to the health personnel, 90% knew the Initiative, 72% took a Breastfeeding course and 40% expressed that there are obstacles. Out of 49 postpartum women, 57% were informed about breastfeeding, 77% knew how to take care of their nipples but didn't know the nursing frequency. Exclusive breastfeeding in infants under the age of 6 months was of 11.11%. Conclusions: Since the Initiative was implemented at Dr. Lucio Molas Hospital, admissions of neonates presenting breast milk jaundice were reduced. Seventy-two per cent of women are qualified to achieve an effective breastfeeding. Discontinuity in the training in 2009 is noted in presence of untrained actors. This helps to keep staff resistance to implementation of the Strategy and is reflected in the non-fulfilment of goals exclusive breasfeeding of the sixth month.
